What this notice covers

This notice covers the websites happyurology.com and its Spanish companion urofeliz.com. It is the website privacy notice; it is not the practice’s HIPAA Notice of Privacy Practices, which is provided at the office and available on request.

Information you send us

Our consultation forms collect your name, contact details (phone, email) and an optional message, for one purpose: so the office can reach you. Please do not submit sensitive medical information (diagnoses, records, images) through website forms — those details belong in your confidential consultation.

Submissions are transmitted securely through our infrastructure providers — Cloudflare (the platform the site runs on) and Resend (which delivers the message to the office email) — and handled as office correspondence. Spam is filtered by Cloudflare Turnstile, which works without identifying you.

So that no request is missed, each consultation request is also passed securely to the practice’s lead-management provider, which places it in the office’s patient-intake system. It receives what you entered (your name, contact details, request type and message), the page you sent it from and, if you arrived through one of our campaign links, that link’s campaign tags. The provider uses an AI service to read the request type and message and write a short note for the office, such as what you are asking about and when you are available. The provider keeps the request for up to 180 days under a business associate agreement with the practice. It is never used for advertising and never sold.

Information collected automatically

To improve the site we use analytics — Google Analytics 4 and PostHog (with everything you type masked). These run by default; you can opt out at any time, and we automatically honor your browser’s Global Privacy Control (“do not sell/share”) signal. We do not load a third-party advertising pixel in your browser. Details, and how to change your choice, are in the Cookie Policy.

Advertising measurement (Meta)

We advertise on Facebook and Instagram. If you reach this site by clicking one of our Meta ads and then send a consultation request, our server tells Meta (the company behind Facebook and Instagram) that a request came in, so we can measure which ads work. That notice contains only: that a request was received on this website (our site’s home address and a random reference number for the request), the time, your IP address, your browser type, and the ad-click identifier Meta added to the link. It does not include your name, email, phone number, message, or the pages you viewed. If you did not arrive from one of our Meta ads, nothing is sent. Nothing is sent if you opted out or your browser sends a Global Privacy Control signal.

What we do not do

We do not sell your personal information. We never give Meta or any other advertiser your name, contact details or message, and we do not use website form data for marketing lists without your consent.

Your choices

Contact the office to ask what contact information we hold from a website submission, or to have it deleted. You can revoke cookie consent at any time from the Cookie Policy page.
