Enlarged Prostate (BPH) Treatment in Miami

A weak stream, constant bathroom trips, getting up three times a night: an enlarged prostate is one of the most common conditions men face with age, and it is treatable at every stage. Is this BPH?

Benign prostatic hyperplasia (BPH) is non-cancerous growth of the prostate that squeezes the urethra as it enlarges. The symptoms build slowly, so many men simply organize their lives around bathrooms for years before asking about it:

  • Weak or interrupted urine stream
  • Frequent urination, especially at night (nocturia)
  • Urgency, or difficulty starting
  • The feeling that the bladder never fully empties

BPH is benign; it is not prostate cancer. The evaluation still matters, because the symptoms overlap and the workup includes a PSA test and examination. If anything about the evaluation raises a separate concern, Dr. Gheiler also offers fusion biopsy and focal therapy for prostate cancer.

The treatment ladder

  • 1. Watchful waiting and lifestyle changes. For mild symptoms: fluid timing, reviewing medications that worsen symptoms, and monitoring.
  • 2. Medication. Alpha-blockers relax the muscle of the prostate and bladder neck to improve flow, and 5-alpha-reductase inhibitors can shrink the gland itself over time. Medication is the right answer for many men; others keep having symptoms despite it, or stop because of side effects.
  • 3. Rezūm water vapor therapy. An in-office procedure that uses the natural energy of water vapor (steam) to shrink the tissue blocking urine flow, for men who want relief without major surgery or a lifetime of daily pills.
  • 4. GreenLight laser therapy. Photoselective vaporization of the prostate: a laser vaporizes the obstructing tissue and seals blood vessels as it works, which makes it well suited even to men on blood thinners. Usually a same-day procedure.

Which one is right for you?

It depends on your prostate size and anatomy, how severe the symptoms are, your other medications, and what matters to you, including how each option can affect sexual function. Dr. Gheiler evaluates all of it and tells you plainly which options fit your case and which do not. He has been designated a GreenLight Center of Excellence by Boston Scientific since December 2020, and offers Rezūm in the office.

Many of Dr. Gheiler’s patients see him for both BPH and erectile dysfunction. If you have both, one urologist can treat both, and plan the treatments so they work together.

GreenLight laser therapy for diagnosed BPH is generally covered by Medicare and most major insurance plans, and coverage for the other options is verified at consultation before anything is scheduled.
